• Provides an accommodation allowance of $100 per eligible trip to patients whose closest specialist or designated health care facility able to provide the required services is at least 200 km one-way. 
• Provides a travel grant of 41 cents per km between your area of residences and the location of the nearest medical specialist or facility

For northern Ontario residents in districts of Algoma, Cochrane, Kenora, Manitoulin, Nipissing, Parry Sound, Rainy River, Sudbury, Thunder Bay, or Timiskaming. Must be referred by a northern physician and the nearest facility is at least 100 km from your area of residence.

If you are also applying for an accommodation allowance, patients must travel at least 200 km one-way. 

1. Get an application form online or from your physician. 
2. Complete section 1
3. Have your physician complete section 2 and 3
4. Include all necessary receipts for accommodation, bus, rail or air travel.
5. Submit to the Claims Services Branch in Sudbury.  

If your application is denied you can request an internal review or reconsideration with 12 months. Appeals information is provided in the denial letter.
